Liposuction 101 

Saddle bags, muffin tops, love handles, banana rolls, meet liposuc-tion.  But, what technique should you choose?  Liposuction, Tumescent Liposuction, Vaser Lipo System, Li-podissolve™, Smartlipo™, etc... Read along and earn a few credit hours as you embark on the road to eliminating bulging fat.

Liposuction

Traditional liposuction is a surgi-cal procedure invented in 1974 that involves anesthesia, an incision, a few hours to perform and several days for recovery. Most importantly, liposuc-tion is best performed by a board certified plastic surgeon and not your dermatologist or your gynecologist. Liposuction is the actual physical removal of fat allowing for dramatic reduction in both circumference and body contouring. Your surgeon in-serts a metal tube, called a cannula, through an incision in the skin and into the fatty area. As your surgeon suctions, the unwanted fat is led to a container attached to a suction pump. Liposuction remains until today the gold standard to shrink specific areas of the body that do not respond to overall reduction in body weight. Re-sults of traditional liposuction, when performed on the ideal patient, far out way any other technique.

Tumescent Liposuction

Liposuction was revolutionized in 1985 with the tumescent technique. Tumescent liposuction involves in-jecting a fluid, mostly a mixture of lidocaine, saline and epinephrine, to expand the fat compartment before cannula insertion. This technique makes suction easier and more effec-tive, but mainly contributing to less bleeding and bruising and eliminating the need for general anesthesia.

Ultrasound & Laser 

Assisted Liposuction

 

Vaser Lipo System, Lipodissolve™, Smartlipo™, Ultra-Sculpt, Pro-Lipo, Lipolite, etc… are all new systems that include cannulas that emit either ultrasound waves or laser wavelengths. The purpose is to further liquefy fat prior to removal. Keeping in mind that best results are achieved when the liquefied fat is suctioned out, the purported benefits to the patient are that fat is easier to remove with less swelling and bruising. While early ultrasound and laser assisted systems led to some side effects such as skin burns and nerve damage, I found that the third generation devices addressed these side effects. These systems re-quire multiple treatments performed within weeks or sometimes once a week, they can be thought of as popu-lar adjuncts to traditional liposuction of post traditional liposuction. Both ultrasound and laser assisted liposuc-tion can be combined.

The gap between the dramatic results of liposuction and the subtle results of the noninvasive, “skin smoothing” ultrasound or laser as-sisted liposuction gets even wider when the added cost of these new systems is considered. From a cost perspective to the patient, the multiple sessions required with the noninvasive approach can add up to the cost of one traditional liposuction.

If you are overweight, you need a lifestyle change and not liposuction regardless of the tech-nique used to perform the liposuc-tion. You may benefit from combo techniques such as liposuction and abdominoplasty (tummy-tuck) if you have excessively loose skin. If you are not overweight but have those unattractive lumps around your hips, thighs or buttocks then liposuction is just what the doctor ordered.

So what’s a patient to do? Do your research, be a well-informed patient, set realistic expectations and most importantly choose your experts wisely and then, be on your way to perfect curves.

Breast Size Matters

To women in their 20s, 30s, 40s and even 50s, breast size is no small issue.  First comes the decision to join the ranks of the “surgically endowed”, then comes the harder choice of getting the size right.


With the abundance of preconceived negative ideas about electing to have plastic surgery, to join or not to join the ranks of the surgically endowed becomes an issue of self-esteem and plain human sexuality.  With the number of women undergoing breast augmentation increasing in staggering percentages year after year, understanding the benefits that occur from undergoing breast augmentation has been the subject of many studies. Numerous studies have been conducted showing a direct correlation in the improvement of women’s self esteem and sexual satisfaction and an absolute psychological improvement following breast augmentation surgery. These studies show that patients seeking breast augmentation merit respect, compassion and understanding instead of dismissal and stereotyping.
Ideal patients are women who do not view plastic surgery and breast augmentation as the end-all cure to their sexual afflictions and self esteem.  An ideal plastic surgeon has the ability to screen patients with serious underlying psychological issues and elects not to operate on patients who feel that their life will completely change after plastic surgery.  The combination of the ideal patient with the ideal plastic surgeon leads to what the doctor ordered, “Just the right size breast” from the first time.


Augmented breast have a spherical shape with an outward projecting areola and nipple, a rounded contour, narrow base with a wider cleavage and fullness on the top of the breast.  Enhanced breast have a natural more youthful appearance when the size is right.  The one decision about breast augmentation that often goes wrong is size.  A size satisfied patient is a patient who feels confident about her ability to evaluate her own plastic surgeon’s ability and advice.  Factors such as the 34C ideal cup size stigma, ill advice from friends, the wrong surgeon or the wrong perception of the ideal final results can lead to dissatisfied patients. In my twenty years of consulting patients in the field of plastic surgery, and in a practice with over two thousand satisfied breast augmentation patients, I have been able to accumulate a few pearls in order to help you get it right the first time on  “breast size matters”:

    •    Let your body structure dictate the cup size and not some celebrity chosen implant size.  This is not a one size fits all matter.
    •    The implant size your best friend chose might not be the size you should blindly choose
    •    Talk with your surgeon about your goals and base your goals on your original breast size and shape
    •    Check out your surgeon’s own patients before and after photos
    •    Check out the nuances that separate your plastic surgeon from the next and make sure you like his philosophy towards your procedure, as well as his personal and professional profile
    •    Check out your surgeon’s artistic ability. Can he or she draw or illustrate your procedure and your options or rely on computer generated images.  A plastic surgeon with no artistic talent is like a painter with no brush.
    •    Get a breast sizing consultation and try on different size implants or call us for our -at home pre-consultation kit-
    •    Most dissatisfied patients regret going too small, so whatever size you finally decide on, going a little larger will make you happier in the long run.
