In my previous articles in Your Health, I took a look at the 10 cosmetic plastic surgery predictions of The American Society for Aesthetic Plastic Surgery (ASAPS) for 2011. In this article, I’ll provide insight regarding which of these predictions came true for us at Luxxery with Plastic Surgeon Dr. Hakki.

Kardashian, Beyonce, and Jennifer Lopez have made a shapely rear-end a must have accessory. In the coming year patients will be seeking posterior body lifts, buttock lifts, surgical and nonsurgical buttock augmentations to shape and augment their buttocks.” – ASAPS 2011 Prediction.

As I take a look back on 2011 at Luxxery, Dr. Hakki’s practice is consistent with the above ASAPS prediction. Further analysis shows a marked incline in gluteal augmentation surgery. The Brazilian Butt Lift or buttock augmentation procedure by way of fat grafting is at the top of our list for patients requesting body contouring procedures. Dr. Hakki’s practice has shown a threefold increase in the Brazilian Butt Lift procedure as compared to the year before. Although cultural differences are a factor that plays a role in what is considered trendy, it seems that the idea of having a larger backside bypasses ethnic, cultural and racial variances. Our patients have learned to prioritize procedures that make them the happiest while simultaneously managing their budgets.

The Brazilian Butt Lift has not only increased in popularity in Dr. Hakki’s practice, but also all over the world. Unfortunately, the increased demand and heightened popularity of this procedure has patients seeking other options such as artificial products. These unsafe procedures such as liquid silicone injections in the gluteal-buttock region are unethical and dangerous. Injectable silicone is not a medical product. It is, however, an industrial product. It can never become part of the body, and can cause abscesses and infections. Silicone is also prone to scarring and can end up extruded through the skin from the body. Silicone gluteal injection procedures are being performed by criminal, unlicensed individuals who pose a danger to the health of their patients.

Now let’s discuss the healthy and medically approved way to increase the size of your butt. A Brazilian butt lift by way of fat grafting involves taking fat from one area of the body and injecting it into the buttock region. The fat is obtained by way of liposuction of a patient’s abdomen, flanks, bra rolls or any other areas. The fat grafting process transfers the fat into the buttocks area. The grafted fat is the ideal filler for injection because it comes from the patient’s body with no immune factors. The procedure provides patients with safe, long lasting results. This is referred to as “autologous” fat transplantation, a technique considered by many as the choice procedure for buttock volume enhancement.

The Brazilian butt lift uses a technique that harvests fat rich with living cells. The fat is placed into the recipient’s buttock tissue, where the body houses large veins to insure its survival. Dr. Hakki then creates multiple tunnels where he places microdroplets of fat allowing for the deposited fat to be exposed to the surrounding tissue. There are no disadvantages to the Brazilian butt lift or any fat grafting procedure if it is well executed. The procedure is technique-dependent and Dr. Hakki’s advanced training and experience in fat transfer has allowed his patients to enjoy truly successful results.

 How can I be an artist at heart, someone who celebrates the human form, and be in the business of changing it? I thought for a long time that a plastic surgeon’s job was to make you look better. A plastic surgeon’s job is to make you feel better. The only way I can make you feel better is by making you look like you think you look. My first year at Georgetown, an NIH psychologist presented a study on plastic [surgery]. She found that people with the most confidence are those whose body image, what they see in the mirror, is very close to their self-image — what they see when they close their eyes at night. If I can achieve an approximation of these two things, then I’m going to make you feel better, and making you look better is secondary. Women want to control their own destiny — that’s all. The last thing they want is another man telling them how to look. 


This obsession with looking good? Plastic surgery is a follower, not a leader in this. Women’s need to look better is driven more by celebrities, the media and cameras in cellphones. Every day, I see young women taking photos of themselves, looking at it and shaking their heads. They don’t need Botox, they need a better camera. My next career will be a plastic surgeon to pixels. 

At least once a week someone says, “You probably look at people and always see their faults.” Not true. I look and think: If they were a painting, what could I do to bring out their beauty, make them look a little better? I’m luckier than Michelangelo. When he was chiseling away at David, he made his bicep a little bigger and his nose a little smaller. I bet Michelangelo would have loved to make the model, not the stone, look better. 

In Waldorf, where my first practice opened, [a lot of] women [are] walking the streets with my breasts in them. I pretend I don’t know them. Sometimes they call and say, “We saw Dr. Hakki at Captain Billy’s. Why is he so stuck up when he’s so nice in the office?” I don’t want to embarrass them. If someone passes you on the street and can see my breast augmentation, my Juvederm, my butt lift, I have failed. Your own mother should see you and have no idea. 

I say no every day. When a woman says, “My husband likes big boobs,” or “I want to save my marriage,” I will not do it. If you are changing who you are for someone else, I will not help. When a patient says she woke up with a bad feeling in her gut, I’ll cancel the surgery. I’m still superstitious; the artist always controls the scientist.

Botox party at D.C.'s LuxxeryExpress: From 'Boytox' to 'Blowtox'

Last night, D.C.'s first walk-in Botox clinic opened its doors to the wrinkled. Dr. Ayman Hakki, the cosmetic surgeon behind Glover Park's new Luxxery Express Botox Boutique, aims to inject the popular muscle-freezing protein into area faces without the hassle of a doctor's appointment. After all, hassle leads to frowns. Frowns cause wrinkles. And wrinkles require Botox.

Perhaps it was the heavily-trafficked open Champagne bar; perhaps it was the carefully blocked nerve impulses. For whatever reason, few frown lines were on display at Luxxery's grand opening fete yesterday evening. "Everybody's in a good mood here tonight," Hakki's 81-year-old mother, Maha, told me. "It's the idea of plastic surgery. It makes people happy."

BROTOX.

"I'm 56 years old. I'm a child of the '50s, baby," says Hiba Bittar, Luxxery's chief operating officer and Hakki's wife of 33 years. Bittar, playing hostess tonight in a tight floral dress, looks neither young nor old. She agrees to give me a tour of Hakki's work.

"Obviously, at 56, I wouldn't have these breasts. I have breast implants and the whole world knows it," she says, clutching a Piper-Heidsieck Champagne cocktail in one hand. Bittar sweeps her free hand over her face. "And I do all the typical fillers and fat transfers that you need to do to look like I do."

And her husband performs it all for her. "Oh, he's operated on every family member of mine and his," Bittar says. "Our son, nieces, brothers, brothers' wives. His mom." Call it "Brotox."

Hakki's mother, Maha, received a face-lift from her son at age 55. Now 81, Maha doesn't do Botox. "I'm OK! I'm fine! I don't need it!" she tells me.

She gets a different kind of benefit out of Hakki's cosmetic treatments. "I have the in-law suite," she tells me. "Bedroom, bathroom, sitting room, living room, everything. I've got my own car. I drive places." And she gets invited to the parties.

BOYTOX.

"Botox knows no age, no gender," Bittar tells me. In fact, Luxxery's first male client walked in just the other day. Bittar estimates that 20 percent of Luxxery's Botox recipients are male, but that most of them "wouldn't want you to know they were in here."

Bittar has got a plan to change that. "I'm going to call it 'Boytox,'" she tells me. "Men shouldn't have to feel weird about doing what they need to do to take care of themselves."

The men in attendance are split on "Boytox."

"No," offers Pierre Rahal, 55.

"Possibly," Don Silvey, 41, says. "It's all about marketing. Why do you think they created Vaseline For Men?"

On a scale of 10 to negative 10, Christien Oliver, 26, rates his interest in Botox a negative 10. If Botox were "Boytox," "I'd still not be inclined," Oliver says. "I guess it might move me to a -9.5"

Timur Tugberk, who rolls into Luxxery in a V-neck t-shirt, blond-dyed mohawk, and sparkly gold slippers, is one of the District's elusive male Botox users. "Love it," says Tugberk, 26. "Necessary."

Tugberk's thoughts on "Boytox"? "That's cute. I mean, it sounds really gay," says Tugberk, who is gay. "But that's OK, gay boys need it too."

What about "Brotox"? "So lame," Tugberk says. "I'm not a bro, I'm a ho. They should call it 'Hotox.' They should call it 'Blowtox.' Now that's a party I'd go to."


BOOZETOX.

Half of the attendees at Luxxery's opening are members of the press. A few are friends of Bittar's from her yoga class at SomaFit. And some are just here for the Peacock Cafe passed radish-and-seawood lettuce wraps, the free samples of Luxxery's private label skin care line, and the open bar.

In the corner of Hakki's clinic, three friends huddle near the gift bags, clutching drinks. They're not here to assess their periorbital lines; they just heard of the party through local business resource Bisnow. One of the guys introduces friend Mary Carrick to me as "a former supermodel. Back in the Stone Age, before you were born." She models "underthings," he tells me. "And shoes."

Carrick denies she models anything. "I'm five-four-and-a-half," Carrick, 44, says. In fact, she says, "I don't really do anything right now." I ask her if an interest in cosmetic surgery brought her out tonight. "Not surgery. I haven't really talked to anyone here," Carrick says. "It's a party. I like to see who's up."

Carrick's friend plucks a green cocktail off a server's plate, splashes the majority of it extravagantly onto Hakki's floor, denies involvement in the spill, and downs the rest. Under the glaring lights of the medical clinic, he asks me why I'm asking so many questions, requests to touch my forehead, and tells me I look like Tin Tin. But he won't give a name: "I like to be discreet," he says.

Liposuction

Traditional liposuction is a surgical procedure invented in 1974that involves anesthesia, an incision, a few hours to perform and several daysfor recovery.  Most importantly,liposuction is best performed by a board certified plastic surgeon and not yourdermatologist or your gynecologist. Liposuction is the actual physical removal of fat allowing for dramaticreduction in both circumference and body contouring.  Your surgeon inserts a metal tube, called a cannula, throughan incision in the skin and into the fatty area.  As your surgeon suctions, the unwanted fat is led to acontainer attached to a suction pump. Liposuction remains until today the gold standard to shrink specificareas of the body that do not respond to overall reduction in body weight and resultsof traditional liposuction remain better than any other technique whenperformed on the ideal patient.

Lipodissolve

Also known as mesotherapy, lipolysis orlipostabil, Lipodissolve™ initially began in Italy in the 1980’s and hasrecently become available in the U.S. The procedure involves a series of injections with the promise ofmelting away unwanted small localized areas of fat.  The chemical cocktail used in the medicated injectionsvaries from doctor to doctor as well as the area to be treated. The mainingredient used in the cocktail, Phosphatidylcholine, is not approved by theFDA .  Phosphatidylcholine is asoybean extract, FDA-approved as an agent that breaks down blood fats but notas a substance for injection to promise fat melting.  Lipodissolve being a noninvasive procedure with thelikelihood of achieving no positive results is has no set regulations on who isallowed to perform it; it is an experimental procedure and has not received theendorsement of  the AmericanSociety of Plastic Surgeons.

Tumescent Liposuction

Liposuction was revolutionized in 1985 with the tumescenttechnique.  Tumescent liposuctioninvolves injecting a fluid, mostly a mixture of lidocaine, saline andepinephrine, to expand the fat compartment before cannula insertion.  This technique makes suction easier andmore effective, but mainly contributing to less bleeding and bruising andeliminating the need for general anesthesia.

Ultrasound &Laser Assisted Liposuction

Vaser Lipo System, Lipodissolve™, Smartlipo™, Ultra-Sculpt,Pro-Lipo, Lipolite, etc…are all new systems that include cannulas that emiteither ultrasound waves or laser wavelengths.  The purpose is to further liquefy fat prior to removal.  Keeping in mind that best results areachieved when the liquefied fat is suctioned out, the purported benefits to thepatient are that fat is easier to remove with less swelling and bruising.  While early ultrasound and laserassisted systems led to some side effects such as skin burns and nerve damage,I found that the third generation devices addressed these side effects.  These systems require multipletreatments performed within weeks or sometimes once a week, they can be thoughtof as popular adjuncts to traditional liposuction or post traditionalliposuction.  Both ultrasound andlaser assisted liposuction can be combined.

The gap between the dramatic results of liposuction and thesubtle results of the noninvasive, “skin smoothing” ultrasound or laserassisted liposuction gets even wider when the added cost of these new systemsis considered.  From a costperspective to the patient, the multiple sessions required with the noninvasiveapproach can add up to the cost of one traditional liposuction.

If you are overweight, you need a lifestyle change and notliposuction regardless of the technique used to perform the liposuction.  You may benefit from combo techniquessuch as liposuction and abdominoplasty (tummy-tuck) if you have excessivelyloose skin.  If you are notoverweight but have those unattractive lumps around your hips, thighs or buttocksthe liposuction is just what the doctor ordered.

So what’s a patient to do?

Do your research, be a well informed patient, set realisticexpectations and most importantly choose your experts wisely and then, be onyour way to perfect curves.

6.    Check out your surgeon’s artistic ability. Canhe or she draw or illustrate your procedure and your options or rely oncomputer generated images.  Aplastic surgeon with no artistic talent is like a painter with no brush.

After seven years at another spa growing it into a multimillion dollar business, Hiba is now integrating plastic surgery, cosmetic dermatology, laser surgery and spa services at Luxxery Cosmetic Medical Boutique while creating a unique and exclusive post-surgery pampering center for Luxxery’s surgery patients. She introduced a complete medical-strength, private-label skin care line as well. Not only is Hiba an asset to Luxxery, but she is also involved in the community, constantly increasing the well being and advancement of women by concentrating on charitable aid and donations to organizations that assist women and children. Hiba is the mother of Dannia, a George Washington University graduate and co-founder of a public relations firm, and Rajai, a former Marine and current student at New York University.
