Brazilian Butt Lift: Future Focus

In my previous articles in Your Health, I took a look at the 10 cosmetic plastic surgery predictions of The American Society for Aesthetic Plastic Surgery (ASAPS) for 2011. In this article, I’ll provide insight regarding which of these predictions came true for us at Luxxery with Plastic Surgeon Dr. Hakki.

“Celebrities like Kim Kardashian, Beyonce, and Jennifer Lopez have made a shapely rear-end a must have accessory. In the coming year patients will be seeking posterior body lifts, buttock lifts, surgical and nonsurgical buttock augmentations to shape and augment their buttocks.” – ASAPS 2011 Prediction.

As I take a look back on 2011 at Luxxery, Dr. Hakki’s practice is consistent with the above ASAPS prediction. Further analysis shows a marked incline in gluteal augmentation surgery. The Brazilian Butt Lift or buttock augmentation procedure by way of fat grafting is at the top of our list for patients requesting body contouring procedures. Dr. Hakki’s practice has shown a threefold increase in the Brazilian Butt Lift procedure as compared to the year before. Although cultural differences are a factor that plays a role in what is considered trendy, it seems that the idea of having a larger backside bypasses ethnic, cultural and racial variances. Our patients have learned to prioritize procedures that make them the happiest while simultaneously managing their budgets.

The Brazilian Butt Lift has not only increased in popularity in Dr. Hakki’s practice, but also all over the world. Unfortunately, the increased demand and heightened popularity of this procedure has patients seeking other options such as artificial products. These unsafe procedures such as liquid silicone injections in the gluteal-buttock region are unethical and dangerous. Injectable silicone is not a medical product. It is, however, an industrial product. It can never become part of the body, and can cause abscesses and infections. Silicone is also prone to scarring and can end up extruded through the skin from the body. Silicone gluteal injection procedures are being performed by criminal, unlicensed individuals who pose a danger to the health of their patients.

Now let’s discuss the healthy and medically approved way to increase the size of your butt. A Brazilian butt lift by way of fat grafting involves taking fat from one area of the body and injecting it into the buttock region. The fat is obtained by way of liposuction of a patient’s abdomen, flanks, bra rolls or any other areas. The fat grafting process transfers the fat into the buttocks area. The grafted fat is the ideal filler for injection because it comes from the patient’s body with no immune factors. The procedure provides patients with safe, long lasting results. This is referred to as “autologous” fat transplantation, a technique considered by many as the choice procedure for buttock volume enhancement.

The Brazilian butt lift uses a technique that harvests fat rich with living cells. The fat is placed into the recipient’s buttock tissue, where the body houses large veins to insure its survival. Dr. Hakki then creates multiple tunnels where he places microdroplets of fat allowing for the deposited fat to be exposed to the surrounding tissue. There are no disadvantages to the Brazilian butt lift or any fat grafting procedure if it is well executed. The procedure is technique-dependent and Dr. Hakki’s advanced training and experience in fat transfer has allowed his patients to enjoy truly successful results.

The media is preoccupied with voluptuous perky buttocks, A.K.A. “Booty Beauty.” I covered all the illegal injectable materials used as alternative approaches for butt augmentations in our February issue of Your Health “Beware: Butt Augmentation.”Allow me to address a new, professional butt lift method. A buttocks augmentation, or “Brazilian butt lift” is a surgical procedure that involves liposuction combined with fat transfer into the buttocks. When aggressive,regional liposuction is performed along with the fat transfer, the procedure becomes the safest and most reliable method for achieving good, measurable,long lasting results. The liposuction portion of the buttocks augmentation procedure is the crucial aspect of the procedure. First and foremost, it is performed using standard liposuction techniques and not ultrasound or laser-assisted.This will ensure the viability of the harvested fat. It is also generally performed on particular areas that compete for attention in the buttocks region. It can do wonders to enhance the appearance of the buttocks. The foundation of all Brazilian butt lifts is the dramatic re-establishment of central fullness.This is achieved by careful planning of areas of liposuction and areas ofaugmentation. The secret lies in liposuction of the flanks, but mostimportantly the gluteus V shelf, which I refer to as the “fanny pack.” Thisarea, the sacral area immediately above the buttock crease, is what sculpts anddefines the curve that extends from the top of the buttock. The extent ofliposuction and amount of fat placement varies from surgeon to surgeon. A greatresult in the Brazilian butt lift is the combination of optimal viableharvested adipose tissue placed near the upper half of the buttocks and thepatient’s preference area.  This will produce a visible ledge on thebuttocks with a soft texture. The procedure can produce dramatic results. Allit takes is a few simple steps.

Consultation:  Meet your doctor. You and your plastic surgeonwill be able to determine whether buttock augmentation is right for you.Patients often have a preferred area for their planned liposuction and have aparticular, ideal buttock shape in mind. While in a standing position, you andyour doctor will be able to look in the mirror. At that point you may decidewhat areas of liposuction you want and where you want fat to be placed for afully satisfying outcome.  To accomplish a smoother silhouette, yoursurgeon may still recommend additional areas of liposuction for optimalsculpturing results. You are not considered a candidate for a Brazilian buttlift if you have unrealistic expectations or a medical history that does notpermit you to have elective surgery.

Surgery:  On the morning of your surgery, you will be asked torefrain from eating or drinking as part of anesthesia requirements. Yoursurgeon will address any questions you might have and mark your operative sitefor the planned surgical procedure while you are in standing position. Youranesthesiologist will describe the anesthesia process and address any questionyou might have.  Your surgery may take 2-3 hours under anesthesia. Differentsurgeons and surgical facilities use different anesthesia methods, yet a BrazilianButtlift can be performed under local anesthesia with intravenoussedation, in which case the patient can breathe independently during the entireoperation. This is the preferred methodsince it is more comfortable. Once the surgery is completed, an abdominalbinder (like a corset) is placed to provide a slight compression to theliposuction area and the patient is transferred to a short stay in the recoveryroom.  Patients usually go home within an hour of recovery.

Recuperation: You will be discharged to a family member or a care-giverwith all your proper medication to begin your recuperation process. Yoursurgeon will give your care-giver your discharge instructions. Otherinstructions include absolutely no cigarette smoking. Smoking will prevent yourhealing and beautiful transformation. You are encouraged to walk to thebathroom and around your house, and a follow up postoperative visit will bescheduled for the next day. It is also recommended that the binder be worn fora minimum of two weeks after surgery.  An ideal recuperation period for abuttock augmentation is one week to ten days or an average of seven workingdays. Some patients can resume their work even earlier provided they do notexert physical activity during the healing period.  Finally, it isrecommended that you resume your normal physical activities gradually in orderto avoid any complications.

Buttocks augmentation is not as simple as it sounds, which is probablywhy it is among the least commonly performed cosmetic procedures in the UnitedStates. Yet in the last two years, the procedure has increased in popularityand has become a commonly researched procedure in cosmetic surgery. There aremany ways you can get a butt augmentation but only a few safe approaches havestood the test of time.

Since the buttocks have a very different anatomy, augmenting themrequires advanced training. How to go about it safely requires refined skillsand a great degree of artistry. A former Miss Argentina made headlines lastyear when she reportedly died from complications of buttocks augmentationinjections. The product used in this unfortunate case is industrial silicone. Thesedangerous “liquids”, named PMMA, or polymethylmetacrylate, or Hydrogel, or Silikon1000, or Aquamid are the subject of this discussion.

Our goal is simple; to educate patients about the potential dangers ofbutt augmentation by injections of unsafe, counterfeit or illegally importedcosmetic injectables. These injections are usually performed by unknown or poorlytrained physicians, or individuals who are not even physicians. Keeping this inmind; the FDA has not approved single filler for buttocks augmentation otherthan fat grafting. Autologous fat is the standard approach to buttocksaugmentation by injections in the U.S. and it is safe when performed byexperienced plastic surgeons.

Liquid silicone injections have more disadvantages than advantages.The product, regardless of what the writing on the label of the vial says, is expensive,with cost ranging between $300-$500 per cc or cubic centimeter. Since 1 cubic centimeter of liquid equals about 0.0338 of oneounce it takes a lot to augment a butt. Cheaper impure versions of the product arealso available but are all suspect. The average butt augmentation requires hundredsof ccs on each side so the process can get pretty costly. Aside from the cost,the bad results sometimes achieved with the product are typically permanentlydeforming. The real danger lies with the complications that may arise later. Thoughthe product initially offers patients sought-after changes its long lastingeffects are bad. The industrial product is not a medical product and the free siliconemolecules in some can result in horrible infection and abscesses that may hardenthe injected site and slough the skin.

It will be hard to find a reputablephysician with experience in butt augmentation using any injectables, sincenone have been FDA approved so far. In the U.S, fat injections, using fat fromthe patient’s own body, is the standard approach to successfully and safelyaugment your buttocks. The procedure is best performed by a cosmetic surgeonwith a vast experience in fat transfer and a proven successful harvesting andgrafting technique.

The message that we are tryingto send to patients is; if you are considering a butt augmentation don’t letsomeone use an industrial liquid on you instead of your own body fat?

If you must, then beware…

Ask the following questions:  

·      Is the practitioner board certified by the American Board ofPlastic Surgery or any other board that trains physicians in cosmetic surgery?

·      Is the product approved by the Food and Drug Administrationfor butt augmentation? And what is the physician's success rate doing this.

·      Are your results permanent?

The answers to all abovequestions can best be addressed by a board certified plastic surgeon. If his orher practice includes buttock augmentations he or she can give you a buttocksaugmentation in a safe manner, with permanent results, using your own body fat.
